Background: Current literature warrants surgical decompression in paraplegic dogs with absent pain perception (APP), but the rate of ambulatory dogs with APP following thoracolumbar (TL) IVDE surgery in a clinical setting remains unknown. Furthermore, the outcome of paraplegic APP French Bulldogs (FBs) is anecdotally considered poor.The aims of this study were threefold within a large population of TL-IVDE paraplegic dogs with APP undergoing decompressive surgery: (1) to characterize early spontaneous pelvic limb movement and ambulation following surgery; (2) to identify risk factors for the recovery of ambulation; and (3) to compare the outcome of FBs and Dachshunds presenting with APP.Methods: A single-center, retrospective case series of dogs with paraplegia and APP diagnosed with TL-IVDE based on CT or MRI, all undergoing surgical decompression (hemilaminectomy ± durotomy). Two main groups were defined: ambulatory and non-ambulatory. These were further characterized depending on the presence of pain perception and spontaneous movement.Three studies reporting on the outcome of paraplegic dogs with absent pain sensation following SCI and undergoing intensive physiotherapy revealed that 37–59% of cases developed spinal walking, in a population of dogs of TL-IVDE alone or alongside traumatic dogs with surgical or conservative management (15–17).

Therefore, normal test results do not rule out the possibility of pancreatic inflammation.Administration of oral pancreatic extracts does not affect serum TLI concentrations in either normal dogs/cats or those with EPI. Therefore withdrawal of enzyme replacement therapy prior to testing of dogs/cats that are already receiving supplementation is unnecessary.Additionally, assays of serum cobalamin (vitamin B12) and folate are strongly recommended whenever serum TLI is assayed. Affected dogs are managed medically or surgically and, although outcomes are usually successful, substantial nursing challenges remain.Thoracolumbar IVDE has been recognized since the 1880s and is the most common cause of acute paraparesis and paraplegia in dogs because of the popularity of chondrodystrophic dog breeds. 4 , 5 As a result, there is much literature available, the majority describing observational studies. Notably, guidelines on management of the condition are lacking. In this consensus statement on TL‐IVDE, we sought to review the available literature, weigh evidence, and generate recommendations for management and highlight areas in need of study.A Qualtrics survey of the American College of Veterinary Internal Medicine (ACVIM)‐Neurology membership was distributed in October 2020 to gather priorities related to a consensus statement on diagnosis and management of acute TL‐IVDE in dogs. Eleven topics were identified, and subtopics were developed within them (Table 1).

Prophylactic fenestration of the offending disks is routine in TL IVDD surgery, and many surgeons fenestrate adjacent, dehydrated disks in the area.23 In the cervical spine, prophylactic fenestrations of the intervertebral disk C2-C3 through C5-C6 are routinely performed in chondrodystrophic dogs in conjunction with ventral slot decompression.24 In 1 publication, the recurrence rate of IVDEs seems particularly high in the French bulldog,25 perhaps giving another reason for prophylactic fenestrations in this breed.Other authors have cited increased morbidity with the use of MPSS, usually when given in much higher doses over the course of 24 to 48 hours30,31; however, Olby et al cited that many of the dogs in these studies had also received NSAIDs.32 In this particular study, which evaluated the use of MPSS in dogs without nociception, Olby et al stated that adjunctive medical treatment of surgically decompressed acute TL intervertebral disk herniation with MPSS is safe with use of the protocols outlined, but the study did not find a benefit in the deep pain–negative dogs evaluated.32 The use of gastrointestinal protectants is always advised when using this drug,31 and MPSS should never be used in conjunction with NSAIDs in dogs.One recent publication suggests that the combination of an extensive hemilaminectomy decompression and durotomy may improve the chance of recovery in dogs with TL-IVDEs and absent nociception26; however, other papers have not shown similar results.27,28IVDD is the most common clinical spinal disorder in dogs.
